L'idea di DELTARUNE arrivò a Toby Fox nel 2011, durante l'università, in un sogno fatto con la febbre: vide il finale di un videogioco e decise che doveva crearlo. Un'altra ispirazione furono alcune illustrazioni di carte da gioco pubblicate su Tumblr dall'artista Kanotynes. Fox iniziò a lavorarci nel 2012, ma abbandonò il progetto prima ancora di realizzare la prima stanza.

Parte della musica composta allora finì in Undertale: per esempio il tema di battaglia diventato «Bonetrousle». Durante la campagna Kickstarter di Undertale, nel 2013, Fox decise che, se avesse terminato quel gioco, sarebbe tornato a DELTARUNE unendo l'idea originale al mondo di Undertale. Il titolo stesso è un anagramma di Undertale.

Grafica migliorata, combattimenti rinnovati e i nuovi impegni dovuti al successo di Undertale resero lo sviluppo più impegnativo. Il gioco esce a capitoli, e Fox ha paragonato i primi quattro all'equivalente di «due Undertale e mezzo».
